Statewide HCBS Medicaid Waiver
The Southwest Area Agency on Aging & Disability serves as the administrative lead agency under the Medicaid Waiver component for its planning service area. To be eligible, an enrollee must:
- Be a resident of the State of Tennessee.
- Meet the financial requirements to receive Medicaid as determined by the Department of Human Services.
- Meet the TennCare pre-admission evaluation (PAE) medical criteria for level I nursing facility care.
- Be an adult of 21 years of age or older.
- Have a home that is a safe place to get Waiver services to protect the enrolleeās health and safety.
- Have an identified caregiver.
Services provided include case management, homemaker services, personal care services, minor home modification, personal emergency response systems, home-delivered meals, inpatient respite care, adult day care, assisted living, assistive technology, in-home respite, personal care attendant and pest control.
